We are looking for a Hospice Patient Care RN Coordinator for the HCN Sonoma Hospice Indirect Department at St.

Summary:
Provides oversight and coordinates daily patient care services for an entire program's patients, serving as a resource to staff, patient/families, physicians, vendors, hospital staff and insurance companies.
Coordinates team assignments with the Supervisor of Patient Care, and Supervisor of Counseling Services.
Provides support for staff and ongoing coordination of information between team members, field staff and office staff.
Oversees the functions of an interdisciplinary team (IDT) and coordinates staffing schedule for nursing services.
Essential Functions:
Oversees and coordinates nurse staffing to meet requests for care and quality staff performance within budgeted hours.
Provides leadership in facilitating IDT delivery of care, care plan integrity, and problem solving.
Knowledge of pain and symptom management, infusion therapies, respiratory and nutritional therapy.
Oversees the Plan of Care for patients and families.
Utilizes policies and procedures to guide delivery of care and daily operations.
Effectively supports staff; skills include active listening, time management and problem solving.
Knowledge of hospice insurance coverage, reimbursement and benefit management.
Knowledge of the regulatory requirements for providing care to patients in all settings (e.
g.
home, SNF, Assisted Living) and the difference between these settings.
Demonstrates sound understanding of and commitment to the hospice philosophy of team and care.
Enforces safety policies and procedures and safe practices by staff and volunteers.
Skills :
Good verbal and written communication skills.
Good computer skills.
Demonstrates knowledge of community resources.
Ability to provide team leadership.
